Any current medication you are taking
Information relating to the assessment and treatment of your problem
This information is provided by you and is used to generate a patient record. This is stored securely and will also contain any medical documents that you have provided us, physiotherapy treatment notes and forms you have completed. We will never pass on your details to a third party without your consent.
Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are:
Your consent. You are able to remove your consent at any time. You can do this by contacting us at the above address.
We have a legal obligation. We must keep your health records for an eight-year period. After this it will be securely deleted.
